Complete the (K-12) Dynamic Form* - STUDENTS ONLY
First time submitting the Dual Enrollment Approval (K-12) Dynamic Form? Please watch the Video Tutorial and Step-by-Step Guide to help you through creating your username and password as well as completing the form.
You must have your College Student ID number, your parent's email address, your HS Counselor's name and email, the class(es) you wish to enroll in, and username and password (If this is your first time using a Dynamic Form, you will need to create an account).
*PARENTS & COUNSELORS: You will be required to create an account to sign the K12 Dynamic Form. The link is different for parents and Counselors. Please watch the Video Tutorial for Parents or the Video Tutorial for HS Partners to guide you through the process.
Follow up with your HS Counselor to confirm all paperwork was processed.
