If the Financial Aid Office has requested that you meet with a counselor to complete a petition		If you wish to request a letter of recommendation for a scholarship application
Reinstatements (yellow form) are completed on a walk-in basis Extensions (beige form) are completed on an appointment basis If you are unsure of which petition you need to complete, please contact the Financial Aid Office at (818)364-7648		It is recommended that you request a letter from an instructor or counselor with whom you have an established relationship You must schedule an appointment with a counselor no later than 2 weeks before the application deadline Please bring a personal statement (if applicable) or a brief statement about why you are applying for the scholarship and the scholarship application
