Official Transcripts

Official transcripts may be requested in person at the ITV/The Weekend College Office, Room 1528, Campus Center, LA Mission College, 13356 Eldridge Ave., Sylmar, 91342. The office is open from 8:30-5:00 M-Th and 8:00-4:30 on Fridays. Transcripts can also be requested by Fax, see the directions below.


Step 1: Clear any Holds

Before you order official transcripts, you must CHECK your student account for any holds and PAY any fees owed. If you have holds, your request for transcripts will not be processed.


To check your account for holds:

 

1. Log into the Student Information System (SIS). If you are unfamiliar with the SIS or don’t think that you have an account, you should visit the SIS for instructions on how to log in. Once you log in, click, “View Holds” on the right side of the screen.

 

2. In almost all cases, you can clear holds by making a payment at the Business Office at the college where the hold exists. You may pay by credit card, money order or cash. Check with the campus Business Office regarding their check policy. If your hold cannot be cleared by the Business Office, please contact the appropriate department to clear the hold.

 

3. Do not contact the Business Office with questions about transcripts. The Business Office only collects money for enrollment fees, health fees, or returned check holds. The Business Office does not issue transcripts and they do not know the status of your transcript request.


Step 2: Order your Transcripts

Once you have cleared all holds, you may order transcripts in person or by fax. Official transcripts may be requested in person at the ITV/The Weekend College Office, Room 1528, Campus Center, LA Mission College, 13356 Eldridge Ave., Sylmar, 91342. The office is open from 8:30-5:00 M-Th and 8:30-4:00 on Fridays. The first two transcripts ever requested are without charge.

 

1. Regular Processing – Regular processing is $3.00 per transcript. Transcripts will be processed within 7-10 business days. This does not include mailing time. Transcripts cannot be faxed and are sent by US Mail.

 

2. Rush Processing – Rush process is $10.00 per transcript. Requests received by 1:00 PM Pacific Standard Time will be processed within 1-2 business days. This does not include mailing time. Rush processing is not available for request held for specific actions to be posted.


Official Transcript Request Form Instructions

 

Complete all sections of the request form as indicated below:

 

1. Clearly print your complete name and any other names you may have used while in attendance.

 

2. Provide a phone number where you can be reached.

 

3. Clearly print you date of birth and student ID or Social Security Number.

 

4. If you want your transcript request held until the semester grades are indicate so on the form.

 

5. Select a processing option:

a. Students may receive two transcripts without charge (NOT including rush transcripts).

b. Regular Processing – Regular processing is $3.00 per transcript. Transcripts will be processed within 7-10 business days. This does not include mailing time. Transcripts cannot be faxed and are sent by US Mail.

c. Rush Processing – Rush process is $10.00 per transcript. Requests received by 1:00 PM Pacific Standard Time will be processed within 1-2 business days. This does not include mailing time. Rush processing is not available for request held for specific actions to be posted.

 

6. Provide Credit Card information if paying fee by Discover, VISA or MasterCard. Sign the Credit Card Authorization where indicated.

 

7. Select a delivery option. If you select “Mail to Student,” please clearly print the complete mailing address for your transcript, including zip code. If you are requesting copies be sent to more than one address, a separate request form is required for each address.

 

8. If the transcript request requires special instruction, note them. If the special instructions require a specific form or document to be included with the transcript, include the form.

 

Note: It is the student’s responsibility to ensure that grades are posted when transcripts are requested unless the request indicates special instructions to hold for grades. It is also the student’s responsibility to ensure that there are not any holds on the student record which would prevent the distribution of the transcript.

 

BY MAIL: Complete form. Enclose check or money order payable to ITV/LA Mission College, or provide credit card information and mail to: ITV/The Weekend College, 13356 Eldridge Avenue, Sylmar, CA 91342

 

BY FAX: Complete form with credit card information and FAX to (818) 833-3598.
