Field Trip Forms

	LACCD Field Trip Form
	Student Field Trip Form, please include: 1. Be sure to print the form back to back, it is 2 sided 2. Students must fill out the form in ink, both sides 3. Faculty must turn the form in three weeks in advance of the trip 4. Faculty must wait for approval and an insurance rider before going on any trip 5. Instructors must have a copy of all student forms with them on any trip
	Notification of Off Campus Field Trip Form
	For the Faculty Notification of Field trip, please include: 1. Submit the form three weeks prior to the trip for approval 2. Must have copy with you on the trip 3. Must have authorization prior to the trip 4. Must have an insurance rider issued prior to the trip
	Request for Field Trip Insurance Rider Form
	For the Insurance Rider please include: 1. Instructors must have this on file before any trip 2. Instructors not covered by Workman's Comp without it
